The Gut-Hormone Connection Most Men Never Hear About
Approximately 70% of your immune system lives in your gut — specifically in the GALT (gut-associated lymphoid tissue). Your enteric nervous system contains over 500 million neurons and produces roughly 90% of your body’s serotonin. Your gut wall performs critical endocrine and metabolic functions that directly regulate your hormonal environment.
When the gut is healthy, it does all of this quietly and efficiently. When it’s dysbiotic, inflamed, or permeable, it becomes the upstream driver of hormonal dysfunction that looks nothing like a gut problem on the surface.
Gut-hormone map
The gut does not sit below hormones. It feeds the hormonal signal.
Most men chase testosterone, thyroid, estrogen, and cortisol as separate problems. But the gut can distort all four through inflammation, liver load, nutrient absorption, microbial metabolites, and nervous system signaling.
The Estrobolome
The estrobolome is the subset of your gut microbiome responsible for metabolizing estrogen. In a healthy gut, conjugated estrogen is excreted through bile into the intestine and eliminated. When the microbiome is dysbiotic, certain bacteria overproduce β-glucuronidase — an enzyme that deconjugates estrogen in the gut, allowing it to be reabsorbed into circulation. The result: chronically elevated circulating estrogen, regardless of how well you’re managing estrogen through other means. Elevated estrogen then suppresses testosterone via hypothalamic-pituitary feedback. This is estrogen dominance driven by a gut dysbiosis problem, not a testosterone problem.
LPS-Driven Systemic Inflammation
When tight junctions in the intestinal wall loosen — through stress, alcohol, NSAIDs, antibiotics, low-fiber diet, or chronic infection — LPS (lipopolysaccharide from gram-negative bacteria) translocates into the bloodstream. LPS triggers a systemic inflammatory response, elevates cortisol, suppresses testosterone, increases insulin resistance, and drives the kind of low-grade inflammatory state that makes body composition optimization essentially impossible.
Thyroid and T3 Conversion
Approximately 20% of active T3 production depends on gut bacteria via intestinal deiodinase activity. A dysbiotic gut reduces this conversion. The result is less available T3 — the metabolically active thyroid hormone — with TSH potentially remaining “normal.” This is a mechanism for thyroid-pattern symptoms (cold, low energy, slow metabolism, hair thinning) in men whose standard thyroid panel appears unremarkable.
Liver-Gut Axis and SHBG
LPS translocation from the gut drives hepatic inflammation through the portal circulation. Liver inflammation upregulates SHBG (sex hormone-binding globulin) production. Elevated SHBG binds free testosterone and renders it biologically unavailable. So the pathway from gut dysbiosis to low free testosterone is: dysbiosis → leaky gut → LPS → liver inflammation → elevated SHBG → reduced free T. Your lab report shows “normal” total testosterone. Your biology is functionally low-T.
Four Gut Patterns Driving Symptoms in Men
You might not spend much time thinking about your gut. Most men don’t. They will say their digestion is “fine” while farting every day, needing coffee to use the toilet, having loose stools, feeling heavy after meals, living with bad breath, or blaming bloating on “just eating a lot.”
That is the problem. Men normalize symptoms because they are not dramatic enough to feel like a diagnosis. But the gut usually whispers before it screams. If you recognize even two symptoms below, treat it as a gut-health signal worth investigating, because that signal can affect every downstream system: hormones, energy, mood, skin, body composition, recovery, and libido.

These are patterns, not diagnoses. Each requires proper testing to confirm. But recognising the pattern is what gets you to the right investigation — instead of blaming everything on testosterone or training.
1. SIBO — Small Intestinal Bacterial Overgrowth
Bacteria colonise the small intestine where they don’t belong, fermenting carbohydrates and producing hydrogen and/or methane gas. Key signatures: bloating that arrives 30–90 minutes after eating (especially starches, legumes, onions, garlic), alternating constipation and diarrhea, belching, nutrient malabsorption (B12, iron, fat-soluble vitamins), and fatigue out of proportion with lifestyle. The gold standard test is a lactulose breath test. Rifaximin is the most commonly used pharmaceutical intervention for hydrogen-dominant SIBO. Elemental diet protocols can also clear the small intestine effectively.
2. Leaky Gut — Intestinal Permeability
Increased permeability of the intestinal epithelium — tight junctions that should be closed become loose, allowing undigested food particles, bacteria, and LPS to cross into circulation. Key signatures: food reactions to things previously tolerated well, systemic inflammation, joint pain without a clear musculoskeletal cause, skin conditions (eczema, rosacea, perioral dermatitis), and persistent fatigue despite adequate sleep. Zonulin is the primary regulator of tight junction integrity and can be measured in stool or serum. Elevated zonulin is a confirmed leaky gut marker.
3. Histamine Intolerance
DAO (diamine oxidase) enzyme deficiency — either genetic or acquired through gut damage, H. pylori infection, or antibiotic disruption — allows dietary histamine to accumulate rather than being broken down in the gut. Key signatures: flushing, headaches, rapid heart rate, itching or hives, gut pain, and anxiety or mood instability following histamine-rich foods (aged cheeses, fermented foods, alcohol, shellfish, avocado). Often misidentified as food allergies. Testing DAO enzyme activity and plasma histamine confirms the pattern.
4. Yeast / Candida Overgrowth
Candida albicans and related species overgrow following antibiotic use, high-sugar or high-refined-carbohydrate diets, or immunosuppression. Candida produces acetaldehyde as a metabolic byproduct — a neurotoxin that directly causes brain fog, fatigue, and mood disturbance. Key signatures: white tongue coating, sugar cravings, fungal nail changes or skin infections, mood instability, and chronic sinusitis. Typically identified through organic acid testing (measuring D-arabinitol) or comprehensive stool analysis.

These are the presentations I look for when the gut hasn’t been considered as the primary driver:

How Gut Dysfunction Blocks Muscle and Recovery
The gym works against gut dysfunction in ways most coaches don’t acknowledge. Training is a controlled inflammatory event — the tissue damage triggers the repair response that builds muscle. When systemic inflammation is already elevated (from leaky gut and LPS), the training-induced inflammatory signal can’t be distinguished from chronic background noise. Anabolic signaling is impaired. Recovery is delayed. The mTOR activation that drives muscle protein synthesis is partially blunted by chronic NF-κB signaling from gut-derived endotoxins.
Nutrient absorption is the second lever. Gut damage reduces the absorptive surface area (villous atrophy in severe cases), impairs active amino acid transporters, reduces stomach acid and digestive enzyme secretion, and compromises fat-soluble vitamin absorption (D3, K2, A, E). A man eating 200g of protein per day through a damaged gut may be absorbing 60–70% of that. The training stimulus is there. The substrate to respond to it isn’t.
The third lever is hormonal. As outlined above: LPS → liver inflammation → elevated SHBG → reduced free T. And dysbiosis → estrobolome disruption → elevated circulating estrogen → testosterone suppression. A man with severe gut dysfunction can have his hormonal environment completely transformed by fixing the gut — before a single hormonal intervention is considered.

Working through the kill phase (elemental diet + rifaximin), rebuilding the microbiome, and restoring mucosal integrity has produced measurable changes in my hormonal and body composition picture that were genuinely not moving before. The thing I’d tell every man who’s hitting a ceiling on hormones, energy, or body composition: get your gut assessed before you add more protocols. Fix the foundation. The upstream driver is almost always simpler than the downstream symptom picture suggests.
The 4R Gut Framework
The 4R protocol (Remove, Replace, Reinoculate, Repair) is the functional medicine framework for systematic gut restoration. It’s a sequence — each phase builds on the one before it.
1. Remove
Eliminate what’s driving the damage or dysbiosis. This means: inflammatory food triggers (gluten, dairy, refined sugars, seed oils, alcohol) for at least 4–6 weeks. If SIBO is confirmed, use targeted antimicrobials — rifaximin (pharmaceutical), or herbal antimicrobials (berberine, oregano oil, allicin) for less severe presentations. If yeast overgrowth is confirmed, antifungal protocols. If histamine intolerance is the primary pattern, a low-histamine elimination trial. Don’t skip this step and jump to repair — you can’t rebuild a system while the damage trigger is still active.
2. Replace
Restore the digestive components that have been depleted or compromised. Betaine HCl with pepsin (stomach acid — most men over 35 are hypochlorhydric, not hyperacid). Digestive enzymes (lipase, protease, amylase) with meals. Bile salts if fat malabsorption is present. These are not permanent interventions — they’re scaffolding while the gut heals and its own secretory capacity is restored.
3. Reinoculate
Rebuild microbial diversity. Multi-strain probiotics — Lactobacillus and Bifidobacterium families for bacterial dysbiosis, Saccharomyces boulardii specifically for yeast overgrowth and post-antibiotic restoration. Prebiotic fiber (partially hydrolyzed guar gum, inulin, resistant starch) to feed beneficial strains — introduced gradually to avoid fermentation-related symptom amplification in SIBO. Fermented foods when tolerated. Microbial diversity is the target — not just high counts of a few species.
4. Repair
Restore the physical integrity of the gut mucosa. L-glutamine is the primary fuel source for enterocytes and the most studied nutritional intervention for tight junction repair — 4–5g daily, fasted. Zinc carnosine supports mucosal integrity and has anti-ulcerative properties. Collagen peptides provide the amino acid matrix (glycine, proline, hydroxyproline) for mucosal regeneration. Butyrate (from fermentation or supplementation) maintains colonocyte health and has anti-inflammatory effects at the mucosal level. This is also where peptide support becomes relevant.

BPC-157 (Body Protection Compound)
The most studied gut-healing peptide available. BPC-157 was originally isolated from gastric juice and demonstrates consistent protective and regenerative effects on the gastrointestinal tract. Mechanism: upregulates growth factor receptors (EGF, VEGF), promotes angiogenesis in the intestinal mucosa, reduces inflammatory cytokines (TNF-α, IL-1β) in gut tissue, and protects against NSAID and alcohol-induced gut damage. Also has systemic effects through vagal nerve activation, which can help break the stress → gut permeability → inflammation loop. Oral administration may be specifically effective for gut tissue; subcutaneous for systemic effects.
KPV (Lys-Pro-Val)
A tripeptide fragment of alpha-MSH (melanocyte-stimulating hormone) with potent anti-inflammatory specificity for the gut mucosa. KPV reduces the release of pro-inflammatory cytokines (TNF-α, IL-1β, IL-6) in colonic tissue through NF-κB inhibition. Particularly relevant for inflammatory gut conditions — including leaky gut, inflammatory bowel patterns, and post-antibiotic mucosal disruption. Oral administration allows direct gut tissue contact. Frequently stacked with BPC-157 for comprehensive mucosal repair protocols.
Glutathione
The master antioxidant, produced endogenously but severely depleted by chronic gut inflammation and oxidative stress. Glutathione directly supports tight junction integrity, reduces intestinal oxidative damage, and is a critical cofactor for Phase II liver detoxification — which is essential for proper estrogen clearance (the same estrogen pathway that becomes dysregulated by estrobolome dysfunction). Liposomal oral glutathione or subcutaneous administration are both viable — IV delivery produces the highest bioavailability. Foundational in any gut-repair and detox protocol.
TB-500
A synthetic version of a thymosin beta-4 fragment, most relevant here as a systemic tissue-repair and inflammation-resolution layer. In a gut context, TB-500 is not the first move and it does not replace the 4R framework. It becomes interesting when gut dysfunction sits alongside poor tissue repair, stubborn inflammation, connective tissue issues, and recovery that stays impaired even after the foundational triggers are being addressed.
